The fishbone diagram helps get the team together and get them to go beyond symptons toward root causes of a problem. Its structure creates balanced view of the topic and ideas for improvement.

It is best used when teh problem has clearly been identified and you want to begin identifying causes, again on the way to the root cause.

Could you provide links to some examples? It's always good to see these things in operation.

I like to use a method called the 5 whys. It's also a very quick and easy way of identifying root cause.
